CFISD ATHLETIC PARTICIPATION PROCESS

Athletic participation in CFISD typically requires an annual submission of a physical and athletic participation forms.

  • All student must have a cleared UIL physical on file with a CFISD campus. The UIL Medical History form must have a detailed explanation if a yes answer is indicated and will be subject to possible further medical evaluation before clearance.
  • ALL PHYSICALS AND FORMS MUST BE TURNED IN TO THE ATHLETIC TRAINERS, NOT THE COACHES.
  • In addition to meeting the physical or medical questionnaire requirement above, all required participation forms (Rank One online forms) must be submitted before a student participates in any tryout, practice, athletic class, open gym, open weight room, athletic competition or to travel with a CFISD athletic team for any purpose.

PLEASE FOLLOW THESE STEPS IN ORDER TO BE CLEARED FOR ATHLETIC PARTICIPATION

  1. Completion of the athletic pre-participation forms, which can be found at:

    • Go to the website listed above to complete your forms. Click the blue button in the center of the page that says “START ONLINE FORMS” and follow the steps as directed. Note: When entering your student ID do not use the “S”, enter only the numbers. (Ex. If your student ID is S123456, you would enter 123456).
    • You must fill out 5 online forms. The 5 forms are:
      • UIL Medical History form (noted above)
      • UIL Signature Page
      • CFISD-Emergency Card Information
      • CFISD-Electrocardiogram Consent
      • CFISD-Field Trip Form
  • The student is required to use the Pre-Participation – Physical Examination Form; NO OTHER Physical Examination Form can be accepted as per the University Interscholastic League rules and regulations.

 Player Expectations

Tennis players must be enrolled in either 6th or 7th period tennis class to be on the tennis team. The only exception to this are seniors who may need graduation credits. Also, Cy-Ridge Tennis players sometimes want to take the tennis class as pass/fail so it will not negatively affect their GPA. Juniors who have taken Tennis I, and Tennis II during their freshman and sophomore years are eligible to take Tennis III as pass/fail. Below are the forms and information you will need to complete this process.

If you are on the team, you are required to participate in after school matches during the Fall season or all day tournaments in the Spring.

If a player has an extreme situation which does not allow them to participate, then they will be excused. However tennis players are expected to be able to make up tests and school work on their own time, and/or make arrangements with teachers. If a player is unable to play without a valid reason, they may receive point deductions in the class and/or may be removed from the tennis program.

Players Driving to Events

In order for a player to drive their own car to tennis events, fill out this waiver and return it to the coach.

Taking Tennis as PASS/FAIL

The 6th period tennis class is primarily focused on beginner tennis. Most players in 6th period tennis are either on the freshman or JV team. 7th period tennis is the more advanced tennis class and is primarily composed of varsity level players. Although when student scheduling conflicts arise, players may be placed in either class.
